An AHA Instructor Records Transfer Request is a formal form used when an American Heart Association (AHA)-certified instructor wants to move from one AHA Training Center (TC) affiliation to another.

Purpose & Why It Exists
- Instructors are affiliated with a specific Training Center (TC) under the the AHA.
- If an instructor changes employment, moves, or otherwise wants to be associated with a different training center, their instructor records must be formally transferred.
- The transfer ensures the receiving TC has full access to the instructor’s history (certifications, renewal records, monitoring, etc.), and that the prior TC no longer claims responsibility.
- It also helps maintain accurate records, compliance, and accountability under AHA’s Program Administration Manual.
The Form & Steps
The Instructor Records Transfer Request is a multi-section form, typically divided into three parts, which must be completed by different parties involved:
- Accepting Training Center (new TC)
- Indicates the new TC agrees to accept the instructor and to maintain the instructor’s records in compliance with AHA standards.
- Instructor (the person transferring)
- Authorizes the release of their instructor records for specific disciplines (BLS, ACLS, PALS, etc.).
- Indicates from which TC (name, ID) the records are being transferred, and to which TC they are going.
- Provides personal contact info (address, phone) for record-keeping.
- Transferring Training Center (current/original TC)
- Confirms transfer of all applicable records.
- Provides signature, TC name/ID, contact address.
- Retains copies of transferred records for a required retention period (often 3 years) even after the transfer.
What Records Are Transferred
The records to be transferred generally include:
- Instructor / instructor candidate applications for the disciplines being moved
- Instructor monitoring forms (performance evaluations)
- Renewal checklists, renewal documentation
- Instructor card(s) issued under the former TC
- Course rosters (within retention window)
- Any compliance or administrative records relevant to instructor status
- Records must comply with AHA’s Program Administration Manual (PAM) requirements
Timeline & Requirements
- The original Training Center is generally required to send the instructor’s files/records to the new (accepting) Training Center within 30 calendar days of receiving the request.
- The accepting TC may need to verify that the instructor has been removed from the former TC’s instructor list in the AHA’s Instructor Network.
- The new TC then issues a new instructor card under its name (once records are verified)
